What is myBeaumontChart?

myBeaumontChart helps you to be an active, engaged participant in your health care. With myBeaumontChart you get secure online access to portions of your Beaumont Health medical record. myBeaumontChart is your personalized view into your medical record stored in Beaumont Health's Electronic Health Record (EHR) called oneChart. With myBeaumontChart, you can:

View your medical record online

  • View your medical information online wherever you have internet access (e.g. your home computer, your SmartPhone, your tablet, etc)...all day...any time.
  • Receive tests results, letters and questionnaires.
  • Review your medications, immunizations, allergies and medical history when your doctor enters them into your electronic chart.

Stay in touch with your Physician

  • Have a question for your Care Team? Send a secure message and forget about playing phone tag.

Manage your appointments and Beaumont Health bills

  • Bypass waiting on the phone and schedule your next appointment online.
  • Request an appointment for an outpatient test at a Beaumont Health facility.

How do I sign up?

You can sign up if you have a myBeaumontChart activation code provided to you during your office or hospital visit. This code will enable you to login and create your own user ID and password today. If you have an access code, please go here to sign up now. Please note: this activation code is only good for 60 days, so don't delay!

What languages does myBeaumontChart support?

By default, myBeaumontChart is in English. We currently also offer a Spanish version. Click the Ver en Espanol link in the upper right corner of any page to switch to the Spanish version.

What is two-factor authentication?

Two-factor authentication is a method of confirming a user's claimed identity by using a combination of two pieces of data. This would be something they know (password) and a second factor (token) in the form of a six-digit number.

Why is two-factor authentication used?

To provide additional security during myBeaumontChart signup, login, and certain other workflows. Two-factor authentication minimizes identity theft and helps prevent disclosure of your electronic health information.

How does two-factor authentication work?

After a patient enters their myBeaumontChart username and password, they are taken to the additional security verification page where they can have a code emailed or sent as a text. After the patient receives the code, they enter that code to finish logging in to their myBeaumontChart account.

When can I see my test results in myBeaumontChart?

Beaumont Health is committed to making your results available to you in a timely fashion. We know that when our patients are actively engaged in their health care, this leads to the best possible outcomes.
ADVISORY:This data could include sensitive information, such as the sex of your baby, HIV, or cancer related results. If you do not wish to know this information or feel uncomfortable seeing it prior to discussing with your provider, please choose to wait to view the results until after discussing with your provider.

The results only include tests performed at a Beaumont facility and timing may vary depending on result type, sensitivity. The current result release guidelines to your myBeaumonChart are as follows:
•Inpatient stay final test results are available immediately at discharge.
•Outpatient activity final test results are available immediately.
•Sensitive results such as cytology and surgical pathology are available 3 days after final resulting.
•Please contact your clinic if you have questions regarding your test result.

If some of my health information on myBeaumontChart is not correct, what should I do?

You have the ability to request updates to various portions of your medical record within myBeaumontChart. For example, you can request updates to your Medications, Allergies, and Health Issues (ake Problem List). Your myBeaumontChart information comes directly from Beaumont Health's EHR. You will see information entered during your visits with Beaumont Health providers, community providers using Beaumont Health's EHR, and results from testing and procedures at Beaumont Health facilities. Information entered by providers or hospitals not using Beaumont Health's EHR will not be automatically included in myBeaumontChart.

How do I request a copy of my medical record?

Access the Health menu from the home page. Select Download My Record. Click on Requested Records and then click the send us a request link. Complete the form that opens up and submit your request.

Can I access my child’s medical record within myBeaumontChart?

You can now view your child's medical information within your myBeaumontChart! You can ask your child's provider for proxy access to your child's records at your next clinic visit or you can request access online. Use this form to request access online. Once you have been granted proxy access, your child's record will appear in a separate tab within your myBeaumontChart, right next to yours.

Yes, you can. myBeaumontChart proxy access allows you to access the medical records of your family members and others you care for, with their permission. You must be at least 18 years old to have proxy access to another person’s medical record.

You might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in myBeaumontChart.

I would like my spouse to have the ability to view my medical records within myBeaumontChart. How can I grant him/her access?

To allow your spouse, family member, or friend access to your medical records within myBeaumontChart you can request this access from within your account. Once you have logged into your myBeaumontChart with your username and password, select Personalize under the Profile menu. Then click the Adult Proxy Request hyperlink within the "Who Can View My Record?" section. Fill out the form to allow access to your medical information within their myBeaumontChart account.

You can also invite someone to have proxy access to your myBeaumontChart account by clicking the 'Health' menu and selecting 'Share My Record'. The 'Friends and Family access' page under the 'Share My Record' section, will allow you to invite another person for proxy access to your account.

I once had access to my child’s myBeaumontChart information and now I do not. What happened?

Once your child turns 18, your access to their myBeaumontChart account is deactivated. Your (adult) child can now request his or her own myBeaumontChart account. If you would still like to have access to your (adult) child, have your (adult) child talk to his or her provider to allow you access, or have your (adult) child apply online through their own myBeaumontChart.

If you're having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the login fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password. You can also contact our myBeaumontChart Patient Support Line at (248) 597-2727 to request assistance.

Can a legal guardian have access to a patient’s myBeaumontChart account?

Yes, proxy access is given to court appointed legal guardians. To do this, provide your legal guardianship documents by submitting them to the Health Information Management Department at any Beaumont Hospital. You can drop them off at any HIM (Medical Record) Department or by faxing them to 248-471-8508. Indicate on your fax that the documents are for myBeaumontChart access. You will then be provided access to myBeaumontChart for the patient until the guardianship documents expire. Continued access will be granted by submitting updated documents.

I just moved here from out of state, and I have never been to a Beaumont before, but my child has. Can I still access my child's records via myBeaumontChart?

Yes, we can grant you access. Ask your child's provider for access, or request it online. Use this form to request access online. You will be sent a letter with an Activation Code to activate your myBeaumontChart account to view your child's records. Follow the instructions on this letter to gain access to your child's myBeaumontChart.

Why is parent/legal guardian proxy access important?

The American Academy of Pediatrics (AAP), along with other medical societies, encourages adolescents to discuss health issues with parents but has supported the right to adolescent privacy. Additionally, Michigan State privacy laws allow certain treatment information to remain confidential between adolescents and their healthcare provider. This proxy access approach to the secure patient portal at Beaumont supports our providers in safely and effectively treating our adolescent patients.

How do I check to see who has full proxy access to myBeaumontChart account and if they have accessed my information through myBeaumontChart?

To view who has account to your account, select ‘Personalize’ under the 'Profile' folder. Each user that is able to view your account will be listed under ‘Who Can View My Record.’

To view who has accessed any information through your online medical record, choose ‘Who’s Accessed My Record?’ under the Health folder. This feature will include who accessed your medical record through myBeaumontChart and when.

    Can I change who has full proxy access to my account?

    •Click the red 'revoke' button if you wish to remove access to your medical records.

    How can I change my password and/or security questions?

    Please click the Forgot Password? link on the login page or click here. Access the 'Profile' menu and select the 'Security Settings' option. You will need to enter your current password to change any of this information.

    How can I change my e-mail address and/or e-mail notification settings?

    Access the 'Profile' menu and select 'Communications'. To change your e-mail, click the 'Edit' button under 'Contact Information'.

    To change any of your e-mail notification settings, simply click the appropriate communication icon next to the communication category you would like to change.

    How can I change my demographics?

    Access the Health menu and select the Demographics option. After editing any necessary changes, click Accept Changes. This will send a request to change your demographic information. After the information is reviewed and confirmed, the demographics will be reflected in your chart.

    Can you send me a new access code as I have lost it, let it expire or did not receive it?

    How do I install the MyChart mobile application?

    Using your apple or android phone, download ‘MyChart’ from the app store. Make sure to accept the license agreement, select the state of your healthcare provider and select ‘Beaumont’.

    How do I setup Push Notifications for the mobile application?

    Log into myBeaumontChart and click on the three dots in the top right corner within the application. Choose Account Settings and then turn on Notifications.

    Where do I see what I owe? How do I make a payment?

    From the home page, click on the View billing summary quick link on the right side of the screen.

    This will take you to your Billing Account Summary. From here, you can see the amount due for various accounts. Click the Pay button to make a payment, or click on the See Account Detail hyperlink to see more information about a particular account.

    What is an E-Visit?

    An E-Visit is a non-urgent, non-emergent online visit initiated by the patient. Use the Submit an E-Visit quick link on the right side of the screen to get started. Read through and accept the E-Visit disclaimer and you've started the workflow. Select from a predefined list of reasons for your E-Visit and select the provider you want to send your E-Visit to. Note that your recipient list will be determined by which providers are accepting E-Visit messages and your relationship with said providers. Not all providers accept E-Visits at this time, thus you may or may not see your care team of providers listed. Follow the rest of the screen prompts and you're on your way to receiving virtual care!

    How much do E-Visits cost?

    Depending on the complexity, an E-Visit could range from $45 - $160. We will bill your insurance carrier for these services. You will be financially responsible for any charge related to the E-Visit that are not covered by your insurance carrier and any follow-up care ordered or directed as a result of the E-Visit, including, but not limited to, diagnostic test, prescribed drugs, laboratory test, referrals for subsequent care or an office visit.

    What is a Video Visit?

    Video visits offer you the ability to receive non-urgent care from your provider through a live audio/video stream using your smartphone, tablet, or computer with webcam. To schedule a video visit, simply log onto myBeaumontChart, click the Schedule an appointment quick link on the right side of the screen, and choose the "Video Visit" option. From there, you will be prompted to provide the information necessary to schedule the video visit. Currently, the following browsers are supported: Internet Explorer, Firefox, Google Chrome, and Safari. Note that not all providers accept Video Visit appointments at this time.
    Make sure you have a stable internet connection on the device you are using for the video visit. If your hardware test is unsuccessful, please contact us at 248-597-2727 for assistance. For any scheduling questions or concerns, please contact your provider.

    What do I do to prepare for a Video Visit?

    We recommend using our mobile app for your video visit:
    1. Download the MyChart App from the App Store (iPhone) or Google Play Store (Android).
    2. Select Michigan and Beaumont.
    3. Select Login to myBeaumontChart
    4. Log in to myBeaumontChart on the App and click on 'Appointments'.
    5. Select your upcoming video visit from the list of visits.
    6. Click 'Test Video'.

    Video visit from laptop or desktop computer with a browser:
    1. Download and install the latest updates to the browser to ensure you have the latest version of the browser.
    2. Open mybeaumontchart.com
    3. Log in to myBeaumontChart and click on 'Appointments'
    4. Select your upcoming video visit from the list.
    5. On the appointment details page, click 'Test Hardware'.
    6. Follow the instructions to download and install the VidyoWeb Plug-in.
    7. After running the installer, close the window and click 'Test Hardware' again. The program will let you know if the test is successful.

    What do I do the day of my Video Visit?

    1. Login to myBeaumontChart on your mobile device or computer.
    2. Click on Appointments
    3. Locate your Video Visit.
    4. Click on your appointment.
    5. Complete the eCheck-In
    6. If it is more than 15 minutes before your appointment, you will see a button to Test Video, otherwise press the Begin Visit button.

    How much do Video Visits cost?

    Depending on the provider/clinic, a Video Visit will likely be between $50 - $60. Beaumont Health will not attempt to bill insurance for Video Visit services at this time. You will be responsible for the Video Visit charge and any follow-up care ordered or directed as a result of the Video Visit, including, but not limited to, diagnostic tests, prescribed drugs, laboratory tests, referrals for subsequent care or an office visit.

    How is myBeaumontChart secure?

    We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal id's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all myBeaumontChart messaging is done while you are securely logged on to our website.

    I was logged out of myBeaumontChart, what happened?

    We aim to protect your privacy and security of your information. While logged into myBeaumontChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of myBeaumontChart. We recommend that you log out of your myBeaumontChart session if you need to leave your computer for even a short period of time.

    What do I need to use myBeaumontChart?

    My access code does not work, what should I do?

    For your security, your access code expires after 60 days and is no longer valid after the first time you use it. If your access code expired, use the Online Access Request Form to gain immediate access.

    Is my access code my user ID?

    No, your access code is not your myBeaumontChart ID or password. You will use this code only once to log into myBeaumontChart for the first time. (The code will expire after you have used it or after 60 days) When you log into myBeaumontChart the first time, you will then be asked to create your own unique myBeaumontChart ID and password.

    What is MyChartCentral?

    MyChartCentral provides you with a central access point for all your MyChart accounts where you can access all accounts using a single username and password. For example, if you also have a Henry Ford Health System MyChart account, you can use MyChartCentral to seamlessly switch between and manage your myBeaumontChart account and your Henry Ford account.

    What is Lucy?

    Lucy is a personal health record that gives you a permanent home to organize all your medical information, regardless of where it came from. You can request an updated copy of your Epic medical record at any time and store it in Lucy.

    You can also add personal information about your health and choose to share it with Beaumont Health and other organizations where you receive care. If your insurance changes or you move away and need to leave your current organization, Lucy will follow you.

    Why am I not finding my other healthcare organization in MyChartCentral?

    Organizations must participate with MyChartCentral in order for you to link to them through MyChartCentral.

    Why isn’t my health information the same at all of my healthcare organizations?

    The information that each of your healthcare organizations knows about you might be different, depending on the type of care they give you and when you were last seen. You can help keep everyone up to date by organizing your records in Lucy and sharing your information among all the different places where you receive care.

    What if I don’t have multiple MyChart accounts?

    Even if you have only one MyChart account, you can still use Lucy as your personal health record. MyChartCentral can provide access to your one MyChart account and Lucy with a single username and password.

    Can I view my family members charts in MyChartCentral?

    Yes, you can! MyChartCentral and Lucy let you see the same family members you can see here in myBeaumontChart. If you’d like to request access to a family member’s chart, please see Proxy Access topic in Frequently Asked Questions.

    What should I do if the information I see in MyChart or Lucy is wrong?

    If information in your myBeaumontChart account or Lucy is wrong, please contact your doctor’s office to update it so your information can be corrected. If information from another healthcare organization is wrong, contact that organization to make sure your information is corrected. After the information looks right in MyChart, you can get an updated copy in Lucy by requesting your chart again.

    What is Share Everywhere?

    Share Everywhere is a new way for you to share your medical information with the people who are taking care of you. Using your MyChart or MyChart mobile account, you can generate a share code and provide it to the person you want to share your health data with. This might be a doctor, chiropractor, physical therapist, dentist, or school nurse, for example. The share code recipient enters that code and your date of birth on the Share Everywhere website to receive one-time, temporary access to your health information. The person who views your information can also write a note back to your health system to help keep your care team informed of the care they provided.

    How long can that person view my record?

    The person you give a share code to can view your health information only until they log out of the Share Everywhere website. They cannot log in later and see it again unless you give them another share code.

    What information can the person I share my record with see?

    The person you share your record with can see a subset of the same information you can see in your own MyChart account, including medications, allergies, health issues, immunizations, results, medical history, visit records.

    How is Share Everywhere secure?

    To generate a share code, you must be logged in to your MyChart or MyChart mobile account. When you generate a share code, it is valid only until it’s used, up to five minutes. To be able to see your information in Share Everywhere, the person you give your share code to must also know your date of birth. If this person enters your date of birth incorrectly three times, the share code is invalidated. As an additional layer of protection, the share code redemption page is protected by reCAPTCHA

    Yes, if you have proxy access to the family member’s information in MyChart. For example, if you have proxy access to your child’s chart in MyChart, you can generate a share code from the child’s record and give it to the provider just like you could from your own MyChart account. The provider can see the same information you can see for your child.

    What is Happy Together?

    Happy Together is a tool that allows myBeaumontHealth to access to health information from other healthcare organizations where you received care and integrate that health information with myBeaumontHealth. It is a feature intended to bring together information from inside and outside of Beaumont Health to form a comprehensive depiction of the care you have received. Connections to other organizations using Happy Together will allow for the health information to be visible by both clinicians and patients.

    How to Link or Unlink external accounts for Happy Together?

    After you log into your myBeaumontChart account, click the Settings menu and select Manage My Accounts. If you have an established relationship with another organization, your account will display. Use the Link or Unlink button to control whether you would like your myBeaumontChart account to display information from the other organizations you have a relationship with.To link your accounts, you must have an active patient portal at another organization using Epic. If you do not see an organization to connect to that you have a relationship with or if you see an organization incorrectly, please contact that other organization.

    What is Open Notes?

    Open Notes gives you easier access to your medical notes through the myBeaumontChart, your electronic health record. When provider notes are readily available, patients are more likely to read them and be more engaged with their health care. With Open Notes, you can also view or print more of your medical record without having to call another office or put in a request for your paper record to be mailed to you.

    While myBeaumontChart has already been your place to see an after-visit summary, test results, appointment information, request prescription refills, or even send a message to your doctor, with Open Notes, you can review more details from your office visit or inpatient hospital stay.

    Why is reading my notes important?

    A lot of information is shared during office visits or during hospital stays. Reading your notes after an office visit or hospital stay may help you remember what you and your health care provider talked about and may help you feel more in control of your care. You may also remember details about your plan of care to help you better manage your health. Reading the notes may not be for everyone. If you are unsure, you may want to start by talking or reviewing a note with your health care provider.

    What may I find in a health care provider's notes?

    After a visit or discussion with your health care provider, he or she writes a note that reflects the visit and summarizes the most important information. The note becomes a part of your medical record. The note may contain:
    •a summary of the information you provided the doctor, which is called history
    •physical exam findings such as blood pressure or how you heart and lungs sound
    •your doctor’s diagnosis and thoughts about your condition or symptoms (this may also be called the assessment or impressions)
    •the treatment plan recommended by your doctor and discussed with you
    •next steps, including upcoming tests, follow up appointments or referrals

    I can't see any of my notes or a specific note I expected to see. Why?

    Possible reasons include:
    •Your health care provider may not use Beaumont’s Electronic Health Record system, called Epic.
    •The note may have been written before June 4, 2019, when sharing doctor’s notes through myBeaumontchart started.
    •The health care provider may not yet be sharing notes.

    Who can see my notes?

    If you have signed up for myBeaumontchart access, you will have access to your notes from office visits or hospital stay as of June 4, 2019. In addition, it is important for you to think about who has full proxy access to your myBeaumontchart account: Any person you have given full proxy access to will be able to open and see the health care provider’s notes shared through myBeaumontchart.

    How will I have access to my medical notes at Beaumont Health?

    As of June 4, 2019, Beaumont will release certain notes to myBeaumontchart once the note has been completed. This may be after your office visit or once you have been discharged from the hospital. Only notes related to visits on or after June 4, 2019 will be released automatically through myBeaumontChart.

    Office visits or hospital stays prior to June 4 will not have notes available electronically through myBeaumontChart, though you may still request copies of those notes through our medical records office.     Where can I find my notes in my account?

    When you view your After-Visit Summary (AVS) on the Appointment Details page in myBeaumontChart, you will notice a Notes tab. The Notes tab displays any notes that were completed and shared by your provider.

    Your note may contain medical language, abbreviations, or terminology that are not easy to understand. If you have questions about your note, write them down and share them at your next appointment, with your health care provider. You may wish to print your note and bring it to your next appointment.     What if I think I may have found an error in my note?

    There are a few ways in which you can address any possible errors in your note:•You may wish to contact your health care provider’s office and discuss the possible error directly with your health care provider at your next office visit.•You may wish to complete a form to request a change to your notes.

    What if my adolescent’s (age 12-18 years old) healthcare provider would like to grant me full proxy access but does not utilize Epic in their practice?

    Once the provider, adolescent, and parent or legal guardian have determined full proxy access is appropriate, they may complete the approval form available at Beaumont.org/proxy Completed forms may be scanned and emailed to MyChart@beaumont.org Contact MyBeaumontChart Support at 248-597-2727 if you have questions or would like additional information.

    What is FHIR

    FHIR is Fast Healthcare Interoperability Resources. It’s a standard that enables applications to retrieve clinical data from Electronic Medical Record systems, such as Epic, using APIs.

    How do I approve a 3rd party application retrieving some of this data?

    If you’ve initiated setup with a 3rd party application which is requesting clinical data from myBeaumontChart (Epic), when you log into myBeaumontChart you will be prompted with a detailed message for approval. The message provides details as to the type of data shared and how it’s accessible.
